Rôle de l’implantation du microbiote intestinal dans la susceptibilité au SII à l’âge adulte : modèle de stress prénatal et infectieux
Le syndrome de l’intestin irritable (SII) est un trouble fonctionnel intestinal (TFI), caractérisé par des douleurs abdominales chroniques, associées à des ballonnements et des modifications du transit. Le SII est responsable d’une importante altération de la qualité de vie (QoL). Cette pathologie est complexe avec une étiologie multifactorielle qui demeure imparfaitement comprise. Le SII est étroitement lié à des comorbidités psychiatriques et psychologiques, comme la dépression, l’anxiété et le stress. La douleur viscérale et les comorbidités psychologiques sont deux variables étroitement corrélées, qui ont comme point commun une modulation anormale des relations bidirectionnelles qui existent entre le tractus digestif et le système nerveux central ; ce qui classe le SII parmi les pathologies de l’axe intestin/cerveau. La description d’une dysbiose, une condition qui fait référence à une atteinte qualitative, quantitative, fonctionnelle et/ou spatiale du microbiote dans le SII a ajouté une nouvelle dimension à ce paradigme. Au sein de notre équipe, nous avons émis l’hypothèse qu’un stress prénatal induirait une prédisposition à développer des altérations de l’homéostasie intestinale à l’âge adulte, similaires à celles retrouvées dans le SII.
Role of gut microbiota implantation in susceptibility to IBS in adulthood : prenatal stress and infectious model
Irritable Bowel Syndrome (IBS) is a functional bowel disorder (TFI) characterized by chronic abdominal pain associated with bloating and changes in intestinal transit. IBS is responsible for a significant deterioration in the quality of life (QoL). This pathology is complex with a multifactorial etiology which remains imperfectly understood. IBS is closely linked to psychiatric and psychological co-morbidities, such as depression, anxiety and stress. Visceral pain and psychological comorbidities are two closely correlated variables, which have in common an abnormal modulation of the bidirectional relations which exist between the digestive tract and the central nervous system ; which classifies IBS among pathologies of the gut / brain axis. The description of dysbiosis, a condition that refers to qualitative, quantitative, functional and / or spatial impairment of the microbiota in IBS has added a new dimension to this paradigm. Within our team, we hypothesized that prenatal stress would induce a predisposition to develop alterations in intestinal homeostasis in adulthood, similar to those found in IBS.
