DESCRIPTION DU PROGRAMME DE RECHERCHE
Le fer, un nutriment essentiel, est nécessaire à de nombreux processus biologiques, mais, en excès, il est toxique. L'absence de mécanisme physiologique pour excréter l'excès de fer rend indispensable la régulation de la quantité de fer absorbée par les entérocytes du duodénum. Cette régulation est médiée par une hormone hépatique, l’hepcidine. L’hepcidine contrôle également la libération de fer par les macrophages qui recyclent le fer et par les hépatocytes qui stockent le fer. L’hepcidine se lie à la seule protéine capable d’exporter le fer connue, la ferroportine, et la dégrade, limitant ainsi la quantité de fer libérée dans le plasma. La transcription de l'hepcidine dans le foie est contrôlée par une interaction complexe de signaux. Elle est augmentée par le fer plasmatique et hépatique afin de maintenir les niveaux de fer corporel stables, elle est également augmentée par l'inflammation afin de limiter la disponibilité de fer extracellulaire pour les microbes et défendre l’hôte contre les pathogènes. Par contre elle est réduite par l'activité érythroïde pour assurer l'approvisionnement en fer nécessaire à l'érythropoïèse. La dérégulation de la production d'hepcidine est la cause principale de nombreux troubles du métabolisme du fer. En effet, une production insuffisante d’hepcidine provoque une surcharge en fer comme dans l'hémochromatose héréditaire et dans la β-thalassémie ne nécessitant pas ou peu de transfusions. Au contraire, une production d'hepcidine trop importante est associée à une anémie comme chez les patients atteints de maladies inflammatoires chroniques ou d’anémie ferriprive réfractaire au fer (IRIDA), et à des dépôts de fer dans le foie susceptibles de contribuer à la progression de la stéatose hépatique non alcoolique (NAFLD) en stéatohépatite (NASH).

La voie principale de régulation de l'hepcidine est la voie BMP / SMAD qui médie l'effet du fer sur l'hepcidine et contribue également à l'induction maximale de l'hepcidine par l'inflammation. Notre équipe a apporté une contribution majeure dans le domaine en montrant le rôle clé du ligand BMP6 dans cette voie. Cependant, un certain nombre de questions sur la régulation de l'hepcidine par d'autres stimuli demeurent non résolues et notre équipe cherche actuellement à répondre aux questions suivantes :
- Quels sont les mécanismes moléculaires exacts conduisant à une augmentation de la synthèse d’hepcidine en réponse à l'inflammation, et par là à la défense de l'hôte contre les microbes extracellulaires ?
- La séquestration du fer dans les macrophages lorsque l’hepcidine est haute est-elle bénéfique ou toxique pour les agents pathogènes intracellulaires tels que Salmonella typhimurium ?
- Par quels mécanismes la matriptase-2, une sérine protéase hépatique, réprime-t-elle l’hepcidine in vivo ? Quels sont la cible matriptase-2 et son inhibiteur endogène ? Quelle est l'efficacité des inhibiteurs de la matriptase-2 que nous avons identifiés pour le traitement de la β-thalassémie ne nécessitant pas de transfusions ?
- Quels sont les mécanismes conduisant à une dysrégulation de l’hepcidine dans la NAFLD ? Quelle est la contribution relative du fer des cellules hépatocytaires par rapport au fer des cellules de Kupffer à la progression de la NAFLD vers la NASH ? Les inhibiteurs de l’hepcidine développés par nos partenaires industriels peuvent-ils empêcher l’accumulation de fer dans la NAFLD et influer sur la progression vers la NASH ?
